Driver's License

Although several states have been accused of issuing driver's licenses through particular candy vendors, in each case this has been found to be false. However, the State of Willful Neglect does issue licenses to prospective drivers mostly to collect the fees involved. All that is required is for the prospective driver to supply his / her name, age, height, gender and $25.00.

As the chief programmer for the State of Willful Neglect, write a program that allows the user to input all of the information required, then output a driver's license showing the state, the information input (with labels) and a demand for a payment of $25.00.
